Once you have sent in the form, you will get it approved and then receive a check in the mail. Covering the costs out of pocket can be an inconvenience, but you should have those costs reimbursed in a short amount of time.
It should be noted that all costs will not be entirely reimbursed. Your deductibles, any copayments, and any procedures not covered will not be sent back to you.
