Ohio Medicinal Marijuana Card Guide
Patients should be aware of the process and requirements before scheduling the appointment to receive an Ohio medical marijuana card online:
Appointments are scheduled online at MMJ.com.
Patients must be 18 years of age or older.
Patients must be a resident in the state of Ohio and have a valid driver's license or state ID
Patients must have one or more medical condition(s) approved by the state of Ohio. See list below.
The total cost of the doctor's evaluation and certification processing fee is $149.99.
There is an additional state registration fee to receive the state issued digital medical card. The additional state registration fee is $50.
Appointments take place online by webcam or telephone call at the appointment time.
If you are not approved, you will receive a full refund of the appointment fee.
The Ohio medical marijuana card is valid for one (1) year from the issue date.
Ohio patients can gain access to a dispensary within the same business day as their appointment. Patient must complete the state registration fee to activate the medical card.
The qualifying medical conditions to receive the OH medical card:
Certified MMJ.com physicians may recommend medical marijuana only for the treatment of a qualifying medical condition. Under Ohio law, qualifying medical conditions include all of the following:

AIDS, amyotrophic lateral sclerosis, Alzheimer’s disease, cachexia, cancer, chronic traumatic encephalopathy, Crohn’s disease, epilepsy or another seizure disorder, fibromyalgia, glaucoma, hepatitis C, Huntington’s disease, inflammatory bowel disease, multiple sclerosis, pain that is either chronic and severe or intractable, Parkinson’s disease, positive status for HIV, post-traumatic stress disorder, sickle cell anemia, Spasticity, spinal cord disease or injury, terminal illness, Tourette syndrome, traumatic brain injury, and ulcerative colitis.
Arthritis, chronic migraines, and complex region pain syndrome. These conditions were determined to be covered by pain that is either chronic or intractable.
