The hard part is figuring out how you tell your ex-girlfriend that you want to get back together with her in the first place. Understanding how to overcome these obstacles is the only way you're going to be able to rekindle your relationship.
The first thing you need to do is determine what went wrong in the first place. This is different with every single relationship, so it's something you'll need to discover on your own. If possible, you should take some time to discuss this with your ex-girlfriend -- but do not put any pressure on her at this point in time. Simply present it as a catalyst for your own self growth for relationships in the future.
Now that you know what went wrong, it is not the time to dwell on things. You do want to fix them, but you need to take some time away from the situation. You should consider this the "no contact" phase. This should last for at least a period of one month. This will give you time to better yourself and prepare yourself to be in a relationship again. It will also give her time to start forgetting the things that went wrong, and start remembering the things she misses about you!
When the time is right, you're going to tell her that you want to get back together with her. Don't jump into this too fast -- this is where many men make their mistake. After you have taken some time off, it's not going to be helpful for you to call her up and say you want to get back together out of the blue! It is far better to plan a reconnection where you meet as friends first. That way you can get to know each other again without any pressure.
Of course, that's not all there is to it! There are certain things you need to do at this point that will help smooth the process, since she moved along more quickly.
